Summary
During rig assembly, a forklift and a crane’s smaller whip line were used to position a derrick board carrying two workers. A chained crossbeam came apart, and the crane line pulled the board off the forks. Both workers fell or jumped; one sustained fatal head injuries after striking a rock.
