Summary
A drill-pipe stand had an incompletely torqued tool joint following assembly in the mousehole. During float-sub make-up with an iron roughneck, the loose connection backed out and the bottom two joints fell, damaging rig-floor handrails before resting against the substructure. No injuries were reported.
