Summary
A person discharged a corroded cartridge-type dry chemical powder extinguisher previously condemned during an annual third-party inspection. Activation of its internal carbon dioxide cylinder was followed by rupture at the corroded base and an explosion. The unit struck the injured person, causing critical head injuries.
