Summary
During scheduled replacement of a potable-water bulk hose on a MODU, a crane lifted one end while the hydraulic reel paid out the hose for deck coiling and backload. The hose separated within a ferrule near the lifting point, recoiled towards the bulk loading station and struck the banksman.
