Summary
Vessel waste was offloaded with flammable paint thinners and potential ignition sources mixed in opaque bags, despite promised segregation. A contractor’s chemist had to open and sort the waste. The flash identifies failures in waste procedures and communication, and records actions on transparent bags, labelling, segregation and crew awareness.
