Summary
A rigger suffered fatal head injuries after falling from a height of three metres during maintenance inside a vessel column, having disconnected fall arrest equipment at an intermediate platform. Pending investigation, the company required task-specific risk assessments and identified matters to consider, including harness suitability, atmospheric testing, supervision and emergency arrangements.
