Summary
A 15 kg flood lamp fell approximately 10 metres from a VLS tower in long-term storage, without causing injury. Its mounting and secondary securing were fully corroded, and storage inspections and maintenance had been absent. Lessons address stored-asset maintenance, dropped-object surveys, possible ancillary-item removal and corrosion-resistant secondary securing.
