Summary
During ROV recovery, a crane wire failed and the vehicle struck a rail before falling inboard. Investigation identified weakening through hook retraction into the sheave and subsequent overstressing during articulation. Instructions address personnel clearance, maintaining wire slack and replacing damaged or overstressed wire at the earliest opportunity.
