Summary
During plough recovery while cable laying, an emergency recovery line snagged on the rudder and was drawn into a thruster after coming free. Loading parted a webbing strop, and winch wires injured two men on deck. Recommendations addressed high-tension slowdown, replacement of the emergency line and lifting-equipment colour coding.
