Summary
A crewman found an exposed live lighting cable sparking against a vessel bulkhead after dry docking. A contractor had removed the flood lamp outside the agreed scope without permits or electrical isolation. Findings highlighted inadequate planning, supervision and communication, and failure to follow management-of-change procedures.
