Summary
An oxy-acetylene cutting hose detached during modification work inside a pile-driving hammer, producing a flashback. An improvised taped connection joined mismatched components and slipped off when warmed. The fire watch extinguished the flames; nobody was harmed. Corrective actions included hose inspections, crew training and renewed emphasis on pre-job checks.
