Summary
This safety flash updates an earlier account of a fatal rescue-boat incident with further MAIB findings. A malfunctioning proximity switch failed to stop the winch during stowage after a monthly drill, resulting in fall-wire failure. The boat and four crew fell nearly 29 metres into water; one died and two were hospitalised.
