Summary
Five people were injured when a fast rescue craft’s lifting lug failed during recovery onto an FPSO following personnel transfer. The craft and occupants fell 11.5 metres. The aluminium lug and support frame were under investigation; the company advised checking lifting-point security through visual inspection, non-destructive testing and weight testing.
