Summary
A fast rescue craft carrying four people began a gradual, unplanned descent after retrieval to a height of 23 metres above sea level. Neutral controls and the emergency stop did not halt it. Both hoist brakes had failed; only the centrifugal/inertia brake arrested the descent. No injuries were reported.
