Summary
A vessel’s moonpool hangar door collapsed after its suspension points failed. Weak design, absent operating instructions and exclusion from planned maintenance were identified; suction was a possible additional contributor to wear. Actions included operating instructions, familiarisation, structural reinforcement and safety lashings to retain the upper mount if overloaded.
