Summary
A 5.6 kg metal panel fell 7–10 metres from an automatic rolling-door hood in an ashore workshop. Rusted brackets, absent secondary retention and a hood excluded from inspection were identified. Actions included examining remaining panels, fitting self-tapping screws as secondary retention, conducting a dropped-object survey and introducing annual hood checks.
