Summary
BSEE investigates a riser bolt insert falling approximately 75 feet during marine riser running on Valaris DS-18, narrowly missing floorhands and damaging steps. Findings address missing retention, incomplete review of work instructions and failure to stop work. Subsequent measures include a checklist, planned staged instruction reviews, step verification and a planned stop-work drill programme.
