Summary
Investigation of tubing dropped downhole during well abandonment aboard the Ocean BlackLion drillship. Findings examine hook-load assessment, hydraulic elevator opening, incorrect relief-valve settings and insulator residue that may have impaired insert engagement. No injuries or environmental impacts were reported. BSEE recommends considering devices restricting loaded equipment opening and a pipe clamp.
