Summary

Investigation of a fire on the Constitution spar platform on 29 April 2017. Compressor vibration wore a hole in coolant tubing against a retaining bolt, releasing coolant onto exhaust heat blankets. Saturated blankets ignited. Operators shut in the platform, extinguished the fire and mustered personnel; no reportable injuries occurred.
