Summary
A ruptured hydraulic line sprayed fluid onto uninsulated turbine exhaust on an offshore production platform, causing a fire. Extinguishers controlled it immediately, with minor platform damage. The safety shut-in system failed to activate. The operator adjusted that system and planned hose, coupling and exhaust-insulation inspections.
