Summary
A Gulf of Mexico production-platform fire followed compressor rod-packing failure and gas ignition at an uninsulated exhaust manifold. No injuries occurred. Findings included a worn, tapered piston rod, improper packing and inadequate vent capacity. The operator planned restoration to factory specifications, increased inspections, resized vents and complete exhaust insulation.
