Summary
Investigation of a synthetic-base mud spill at Shell’s AC 857 reports an estimated 144 barrels released. Findings examine discharge-valve alignment, incomplete job safety analysis, lockout/tagout review and incorrect sheen-test sampling locations. The report records conflicting witness dates, revised fluid-transfer procedures, a planned additional valve and pollution-reporting deficiencies.
