Summary

A platform well blew out after a diverter valve failed to open, the separator line plugged with gumbo and parted, and the annular preventer failed. Personnel evacuated without injury. The alert recommends frequent diverter operation, protected controls, less restrictive diversion where plugging is likely, and planned drilling parameters in severe gumbo areas.
