Summary
Two floormen removing a bell nipple approximately 4 metres above ground were injured after an unexpectedly raised flow line dislodged it. One worker’s lanyard was attached to the nipple; the other fell backwards and remained suspended upside down. Both sustained soft tissue injuries and returned to work the following day.
