Summary
A vessel mast wind sensor counterweight weighing 0.3 kg fell 20 metres after its sole grub screw loosened through vessel vibration. No injury or damage occurred. Previous DROPS surveys missed the single-point failure. The crew fitted two screws with Loctite, modified the second sensor and recommended thorough checks of elevated fixings.
