Summary
A vessel’s gearbox temperature probe was expelled by oil at 25 bar, spraying the engine room and igniting oil-soaked exhaust lagging. Crew contained the fires with foam. The flash identifies an insufficiently tightened compression fitting, describes a redesigned probe housing, and emphasises emergency preparedness and closer monitoring after construction or dry docking.
