Summary
Two stainless steel D shackles failed in a vessel’s life raft release assembly, allowing the container to fall into the water. Recovery using the painter inflated the raft. Possible crevice corrosion prompted replacement of remaining shackles, corrosion inspections, weekly condition checks and improvements to scheduled preventive maintenance.
