Summary
Two crew members were scalded while cleaning a dismantled boiler steam stop valve aboard Queen Elizabeth 2; one later died. Inspection found a leaking isolation valve, thought to have heated trapped condensate until some boiled and escaped. Recommendations addressed steam-line drainage and recognition of safety issues ashore and aboard.
