Summary
Investigation of an aviation-fuel spill during cargo lifting at Garden Banks 189. A generator snagged a fuel tank, which toppled when lowered onto a heaving vessel. Lid and retainer-ring detachment released approximately 390 gallons. Findings address load spacing, uninspected lid fastenings, tank certification and existing lifting precautions.
