Summary
Investigation of a LACT charge-pump fire examines catastrophic motor-bearing failure, axial loading and likely ignition of barrier fluid. Deluge contained the flames, with residual fire extinguished manually. Approximately ten gallons of crude oil remained within the deck drainage system. Proposed measures address pump selection, thrust bearings, barrier fluid and increased monitoring.
