Summary
This investigation examines a fractured foot during permanent well abandonment at VR-214. A manually moved slick-line lubricator rolled out of its stand and was dropped onto a worker’s foot. Findings identify unsupported manual movement and a job safety analysis that failed to recognise manual-handling hazards.
