Summary
Investigation of a finger-crushing injury during preparations to lift a water maker on HI A536 platform C. A worker’s finger became trapped between a sling thimble and shackle safety pin as tension increased. Findings identify the absence of a designated signalman and inadequate job safety analysis for a jammed shackle bolt.
